Chronic Care

The Chronic Care Free Program provides short-term care, up to 90 days, for patients with diabetes, hypertension, asthma, and depression. This program was started in 2011 and is supported by a Nurse Practitioner and RN who provide high quality, cost effective care for chronic conidtions. The ultimate goal of the Chronic Care program is to place the patients with a Primary Care Provider so they will have long term assistance for their disease

Dental Programs

Offered 4x Per Year

The Free Dental Program is a partnership of nearly 20 community agencies with leadership provided from the University of Washington Dental School, with UW dentists and dental students delivering core dental services. The program provides dental treatment to Medicaid-eligible children and their parents. Dental screening and treatment is also provided for special populations, including low income veterans, in a companion outreach program called "House Calls".

With the increased demand of adult care, Community Health Partners has expanded access to dental care to low income adults 60+ years of age, by offering quarterly dental clinic for Seniors

Health Care Advocacy

Working in Conjunction with the Affordable Care Act.

Staff at the Free Medical Clinic are screening every clinic patient to determine eligibility for insurance coverage under the Affordable Care Act. When appropriate, patients are assisted in completion of the on-line process and then assisted in obtaining a primary care provider. In addition, the staff coordinate referrals for patients seen at the clinic who need vision services, specialty care, on-going medical or dental care to providers who have indicated their willingness to participate. This enables physicians, dentists, health systems and hospitals to work within an organized structure to donate services.

Routine Eye Exam

Clinic offered twice a year

(typically offered once in January and once in the summer)

This vision clinic is offered to adults 18+ who have no health/vision insurance. The exam includes a routine eye exam and glasses if needed and will be provided free of charge.
