Wednesday Evenings Walkins 5pm - 7:30pm
Thursday Afternoon Walkins noon - 3pm (starting Feb. 27th, 2020)
The Free Medical Clinic of Community Health Partners provides urgent care services to 18+ uninsured adults. Urgent care services can also offer lab testing, medications and point of service testing, such as basic urinalysis, blood glucose, strep throat, and pregnancy testing for those who are considered appropriate during the patient's visit. Immunizations for seasonal Flu and Tdap (tetanus, whooping cough) are available.
Chronic Care

Thursday Afternoon Appointment or Walkins noon - 3pm (starting Feb. 27th, 2020)
The Chronic Care Free Program provides short-term care, up to 90 days, for patients with diabetes, hypertension, asthma, and depression. This program was started in 2011 and is supported by a Nurse Practitioner and RN who provide high quality, cost effective care for chronic conidtions. The ultimate goal of the Chronic Care program is to place the patients with a Primary Care Provider so they will have long term assistance for their disease
